Can I use warm G.Percha too on suspected tooth surface.

You must be sure that these symptoms are related to this molar, you must perform hot test after tooth isolation with rubber dam and apply hot water path, if the patient complains so the problem here is a missed canal and the cause is the presence of C fibers which can withstand low oxygen tension and responsible for the hot sensation and I had some of these cases,so you must search for this missed canal which could be a second distal or a middle mesial canal and pain on percussion could be due to this reason or due to the periodontal condition added by the abnormal biting habit.

If the patient does not complain from this molar after hot water application so the etiology is from another tooth either upper or lower on the left side, you must diagnose them carefully with hot water test to each one, as the patient has a poor oral hygiene so may an endo-perio problem is the cause for this complain and the pain on percussion on the complaining molar could be due to periodontal or endodontic problems(microleakage due to absence of the coronal filling ) or both, in the early process of periapical infection the X-ray film does not give an accurate data on the real condition.

I have pt whose RCT done in Aug 2007.He came last mth in Jan 2008 with hot sensation in lower left 1st molar,the same I did RCT.i Took Xray no problem as no Infection.He is in habit of taking lot of chalia.V poor oral hygiene.As I did percussion it hurts.No problem in adjacent teeth.No crown over it as Patient does not want.As pt came his filling was also out which I did in Aug 2007.Cavity was filled with chalia.I cleaned the cavity n filled it up with cermet.What shoul I do if he has same symptoms?
